ArenaFysio – Terms and Conditions

ArenaFysio has drawn up the following general terms and conditions for each other’s safety and for a smooth operation. We would like to draw your attention to the following points:

Terms and Conditions

  • Within practice, everyone must behave according to generally applicable standards and values.
  • All registered data is handled in accordance with the provisions of the Personal Data Protection Act (WBP).
  • The practice is affiliated with the complaints procedure of the KNGF.

The First Appointment

  • During your first appointment, you must bring the health insurance card and a valid ID.
  • The first appointment will consist of an Intake & Screening, (physical) examination and a treatment plan will be drawn up.
  • You must agree to the treatment plan so that it can be included in your treatment file.
  • If possible, we plan treatment immediately after the Intake & Screening. This will help you faster.
  • If we cannot help you, we will notify you and advise you to make an appointment with someone else.
  • If we suspect that there is a serious injury or condition, we will determine a next step in consultation with you. This can be a referral to the general practitioner or specialist. This will always be in consultation with you / the parent caregiver when it comes to your child / and the GP.
  • We request you to bring a towel with every appointment.

Direct Accessibility & Referrals

  • A referral is not necessary: nowadays you can make an appointment with the physiotherapist without a referral from the general practitioner.
  • In the event that you have been referred by a GP or specialist, you will need a referral letter

Vergoedingen en Verzekering

  • A referral from the specialist is mandatory for chronic conditions. If you have questions about chronic conditions, please contact your health insurer or physiotherapist.
  • You are responsible for selecting your policy conditions from the health insurer and for keeping track of the number of physiotherapy treatments. It is advisable to check the physiotherapy reimbursement for you before a physiotherapy treatment.
  • If you have changes in your insurance or personal data, you are requested to report this to your physiotherapist.
  • If your insurance does not (or no longer) pay for the physiotherapy treatments, ArenaFysio will send you a bill / invoice. You must pay this invoice within the stated period, by means of the indicated instructions on the invoice.
  • The rates charged by ArenaFysio are fully reimbursed by all health insurers.
  • If the treatments are for your own account, ArenaFysio uses the rates stated on the website.
Cancelation & Missed Appointment
  • If you cannot come to an appointment, we ask you to cancel this at least 24 hours in advance; otherwise we are unfortunately forced to bill you for the appointment. The health insurer will reimburse the missed appointments.

ArenaFysio – House rules

ArenaFysio has drawn up the following house rules for each other’s safety and for a smooth operation. We would like to draw your attention to the following points:

General Rules Calamities

  • Within practice, everyone must behave according to generally accepted standards and values.
  • It is mandatory to listen to the responsible / present therapist, should there be an emergency.
  • If you do not behave properly as a guest at ArenaFysio, the therapist present is responsible and thus authorized to take appropriate measures.
  • In the event of an emergency, you must follow the instructions of the competent authority (fire brigade, ambulance, police).
  • If you would like more information about the rates, payment terms and privacy regulations, please refer to the practice folder on the practice and our website.

Theft & Damaged Property

  • The practice is not liable for damage to and / or loss of property / possessions of visitors located in this building or in the immediate vicinity of this building.
  • The practice does not accept any liability for personal injury to clients who have not followed the physiotherapist’s instructions.
  • If the ArenaFysio patient belongings are damaged through negligence, the patient is responsible and an appropriate solution will be found for articulated material damage.

Patient obligations

  • It is prohibited for patients and visitors to operate the therapist’s laptop.
  • You must be hygienic and washed before every treatment.
  • The use of a towel and the wearing of sports shoes and clothing in the gym is mandatory.
  • There is both a smoking ban and a (pet) animal ban throughout the building.
  • The patient is responsible for arranging health insurance and reimbursements for physiotherapy.
  • If you are unable to attend, this must be reported at least 24 hours in advance via the known channels (telephone number, mail or whats app).
  Unsubscribe & Missed Appointment
  • If you cannot come to an appointment, we ask you to cancel this at least 24 hours in advance; otherwise we are unfortunately forced to bill you for the appointment. The health insurer will reimburse the missed appointments Niet.
